Contract Violation Panel
Angels Service LLC
is dedicated to monitoring results of each client and will conduct regular
service reviews with families, clients, and providers on a regular basis.
Reviews will be conducted in a manner that includes, phone calls, digital, or
in person; the manner in which the review is conducted is at the discretion of
Angels Service LLC or the contracted company who is conducting results
monitoring / investigations.
At any time there is a complaint, report of concern, or contract violation an investigation will take place.
Investigations include:
-A report regarding incident
-Service result check in's with a minimum of 50% of a providers clients
-Compilation of available data for review
-Review by contract violation panel
*A select few contract violations that do not directly involve clients will result in a more automated response with repercussions following the out-of-compliance operating procedure.
Contract Violation Panel
The contract violation panel will include a group of business owners connected to our field, independent consultants, and individuals who may have backgrounds relevant to the current situation.
The contract violation panel will review the contract violations and make a decision based on rules and regulations, policies and procedures, best practices for client well-being, and the available information.
The results will be one of the following:
-No further repercussions other than a violation notice. Future violations will have a more serious outcome.
-Contract termination with a red or yellow rating. Red rating is the contractor is not eligible for a new contract. Yellow rating is the contactor is able to request a new contract after a certain amount of time has passed with stipulations.
-Contract remediation one, contract stays the same with an action plan required. Any further violations of any sort results in an immediate review with possible termination.
-Contract remediation two, contract services are restricted with an action plan required.
Any further violations of any sort results in an immediate review with possible termination.
-Contract remediation three, contract requires an action plan and is under a remediated rate sheet with heightened results monitoring. Any further violations of any sort results in an immediate review with possible termination.
-Contract remediation four, contract is frozen, contract is an under a remediated rate sheet with heightened results monitoring. Any further violations of any sort results in an immediate review with possible termination.
In the event of a tie the more restrictive choice will stand.
